If you collected unemployment compensation collect a 1099-G form from the state agency that paid your compensation and report the amount you collected in line 3 on you 1040EZ. If you collected Alaska Permanent Fund dividends place this value in line 3. If you also collected unemployment compensation add the 2 values together to find out the amount you will need to put in line 3.
